Registered Nurse (RN): Our nurses are compassionate, qualified professionals who provide exceptional care to their patients. Nurses treat a variety of conditions in the home including: Cardiac Care, Diabetic Teaching and Monitoring, IV Therapy, Maternal Child Health, Pediatrics, Wound Care, Psychiatric Care, and Geriatric Care.

The goal of the Home Health Care nurse is to educate the patient and/or caregivers enabling the patient to be safely independent in the home. Patients are able to stay at home longer with the assistance of Home Health Care services. Our nurses manage each case to the individual needs of the patient, identifying additional services needed in the home, and assisting in arranging for those services.

Home Health Aide: Our nurses are assisted by Home Health Aides who help the patient with their personal care needs. Some of these include: Bathing, feeding, dressing, assistance in therapeutic exercises and light housekeeping.

Physical Therapist (PT): Our Physical Therapists provide rehabilitation services, such as gait training, therapeutic exercises, range of motion and other modalities. Our therapists will work with the patient in developing realistic goals toward recovery.

Occupational Therapist (OT): Or Occupational Therapist assists the patient in regaining their ability to carry out activities of daily living, such as feeding, dressing, using assistive devices, and bathing.

Speech/Language Pathology (ST): Our Speech/Language Pathologist can treat a patient with communication difficulties or swallowing disorders

Medical Social Worker (MSW): Our Medical Social Workers assist patients and families with the variety of stresses that can accompany illness. Emotional or financial difficulties are eased with this assistance. Medical Social Workers counsel patients and families and can assist in obtaining financial aide or long-term care.

How to Obtain Home Care

Referrals for home care services can be made by you, your family, your doctor, clergy, or other healthcare professionals. A free in-home evaluation can be made by our staff to assess your needs.

A physician's order is necessary to continue service after the initial visit. Our qualified staff will work with you, your caregiver, and your physician in developing an appropriate plan of care to meet your needs.

Remember, it's your choice when you need health care. Inform your Physician or discharge planner that you have chosen The Greater Bristol Visiting Nurse Association to care for you at home.

Who Pays for Home Care?

Many skilled services are covered by Medicare, Medicaid and other third party insurance. There are certain medical and eligibility requirements that need to be met before insurance approval is made. Our staff will work with you in coordinating benefits. A sliding fee scale is available to qualified individuals without insurance.
