Medicare Certified Hospice Program

What is Hospice?

Hospice is a special kind of program for terminally ill patients who choose to die at home. Our Hospice team, including Hospice Coordinator, Registered Nurse, Medical Director, Home Health Aide, Chaplain, volunteers and Medical Social Worker provide comprehensive and supportive care. This team of dedicated professionals will help you and your family cope with terminal illness. They will use the latest methods in pain management, as well as, their hospice expertise to ensure that patients spend their final days in comfort and dignity.

Who Qualifies for Hospice?
  • Patients, who have a terminal prognosis of six months or less, certified by their attending physician or the Hospice Medical Director.
  • Patients who are no longer seeking curative treatment and have elected palliative care.
  • Patients must have a caregiver in the home, willing to participate in the patient's care plan.

Hospice Mission Statement

The mission of The Greater Bristol VNA's Hospice Program is to provide comprehensive services to caregivers and patients living with a terminal illness.

The Hospice Program maintains dignity while meeting the physical, spiritual and emotional needs of the individual and their family.

The Hospice Program affirms the quality of life as a continuum through an interdisciplinary team approach.

Hospice Objectives
  • To provide for the needs of terminal patients and families 24 hours a day.

  • To maintain the patient at home as long as the environment is safe and appropriate.

  • To provide support services to the patients family/caregivers during the bereavement period.

Payment for Care

Payment arrangements are designed to suit each individual family. Many skilled services are covered by Medicare, Medicaid and other third party insurances. A sliding fee scale is available to qualified individuals.

Hospice Volunteers

Our Hospice program is in need of hospice Volunteers. If you are willing to work with our Hospice team in providing support to our patients and families, please call our Hospice Coordinator at (860) 583-1644 to arrange for an interview.

Bereavement Support Group
